This form is to be completed by the coordinator, organizer or chairperson of the special event and submitted at least sixty (60) days prior to the event. Event Organizer applications submitted less than 60 days prior to the event may not be approved. Failure to receive prior approval may result in closure of the event or other legal action. The operation of the vending site must comply with applicable sections of the Ontario Food Premises Regulation 493/17, under the Health Protection and Promotion Act, R.S.O., 1990 as amended.
