Have you or your child received an immunization? Update your records
According to Ontario law, the Haldimand-Norfolk Health Unit must keep the vaccination record of every child attending day care or school in Haldimand or Norfolk County. The vaccine record is required for school attendance. When your child receives a vaccination at a doctor’s office or a hospital, it is the responsibility of the parent/guardian, NOT the doctor’s office or school, to provide the Health Unit with your child’s vaccination information. The easiest way to report the vaccination information to the Health Unit is through the online immunization reporting form. Information can also be reported to the Health Unit by mailing, faxing or calling using the contact information found on our contact page.
