Naloxone Distribution Program Statement of Need

Thank you for your interest in the naloxone distribution program. In order to participate with the Ontario Naloxone Program (ONP) your agency must complete this form and meet the eligibility criteria. To qualify for the ONP you must indicate that there is a need for naloxone within the population you are serving and that you have adequate staffing capacity within your agency. Once you complete this form, a representative from HNHU Harm Reduction Program will contact you within one week. Return completed forms to [email protected]
