By selecting this link you are able to make a request for your child’s dental/vision screening for the 2024/2025 school year.

For Dental: You may request that your child be included in the screening list to determine if they are eligible for the Healthy Smiles Ontario program or you may request that they be removed from the screening list, therefore, not receive the school dental screening this year.

For Vision: If your child is in SK they will be included in the vision screening this year. If you would like to request they be excluded from vision screening, please fill out the information below. You will be prompted the option to optout for vision screening when you select SK within the Grade dropdown. Note: the option for vision screening only applies to SK students.

Important Note: This request is only applicable for the current school screening year. Any future requests would need to be submitted each year.
